Air Education and Training Command (AETC) has released its Accident Investigation Board report into the loss of a US Air Force F-16C on April 24, 2018, at Lake Havasu City Airport in Arizona – see Attrition, June 2018, p95. The report cites pilot error as the cause of the mishap.
